Abstract:
Risk management activity was designed to assist the practitioner to observe a form of risk and to determine the best treatment to the risk. It is a tool to identify the source of the risk and uncertainties significanly, predict the impacts, and to implement the respond to overcome the risk. The interesting things in this particular research are to identify failure potential and the risk variable impact, determine the most dominant risk, then to specify the strategy to overcome the risk for the fabrication process of hollow core slab using Failure Mode and Effect Analysis method. From the risk identification and the risk analysis occurs 23 failure modes. Failure mode with the highest risk priority number of 168, is the realising-agent spraying which is not evenly distributed on the mold at the mold cleaning process. The strategy to overcome this failure mode risk is to spray the releasing-agent two layers. Meanwhile the most frequent failure mode is the overhead-crane hoist-cable broken off and machine breakdown. The strategy to overcome this failure mode is to employ mechanical engineer to perform schedule routined inspections and regular maintenance.
